Job Description

Under the direction and supervision of the Wound and Ostomy Manager, the Wound, Ostomy, and Continence Nurse will provide safe, competent, quality care based on nursing theory and research to a designated group of patients.

*Key Responsibilities:*
* Conduct health assessments focused on wound, ostomy, and continence care. Perform focused physical assessments and develop care plans based on diagnosis and individual patient needs. Recommend appropriate treatments.
* Assist in the development, revision, and evaluation of care standards.
* Assess and document patient responses to wound care, ostomy, continence, fistula, and complex wound treatments. Evaluate the quality and delivery of care provided by clinical staff.
* Collaborate with the healthcare team and make referrals to other disciplines as needed.
* Educate, counsel, and assist individuals and families in preventing complications related to ostomies, wounds, or continence issues.
* Routinely perform comprehensive assessments of patients at risk for pressure injuries and initiate preventive measures.
* Participate in Unit/Department Quality Improvement (QI) and Hospital-Wide QI activities, such as Incidence, Prevalence Studies, and Nursing Quality.
* Provide culturally competent education to patients, families, and staff, respecting cultural, ethnic, religious, and other belief systems.
* Maintain the security and confidentiality of data and information according to patient rights policies.
* Perform other tasks as assigned.

*Required Skills & Experience:*
* 3-5 years of experience in healthcare.
* Proficiency in using electronic record systems, equipment, and other healthcare and billing systems relevant to this position.

*Required Certifications & Licensure:*
* Active NYS Registered Nurse license.
* Certified Wound Ostomy Nurse (CWON) or Certified Wound Care Nurse (CWCN) issued by the WOCNCB; or Certified Wound Specialist (CWS) issued by the American Board of Wound Management (ABWM); or Wound Care Certified Certification (WCC) issued by the National Alliance of Wound Care and Ostomy (NAWCO).

Pay: $59.00 - $64.00 per hour
